Add a rating field and a textarea (or feedback-labelled text field), and Neural Forms auto-detects it as a Survey. Use multi-step layout for longer forms, add CAPTCHA, configure your autoresponder.

Rating field
Add a 1 to 5 or 1 to 10 star/numeric rating scale. When combined with a feedback text field, Neural Forms auto-detects and classifies the form as a Survey type in your dashboard.
Open-ended textarea
The key field that makes a survey a survey. Any textarea, or text field labelled “feedback”, “comments”, “review”, “thoughts”, or “experience”, triggers survey classification.
Matrix field
Ask multiple statements in one grid. Configure rows and columns with single-choice (radio) or multi-choice (checkbox) mode. Each column carries an optional score for weighted analysis, useful for Likert scales.
Repeater field
Let respondents add multiple entries, e.g. list several experiences or items. Set min/max iterations (up to 20), arrange child fields in 1 to 4 columns, and customize Add/Remove button labels.
Multi-step layout
Break long surveys into digestible steps with “Step” layout blocks. Choose between step numbers, a progress bar, or no indicator, configurable per embed without changing the form.
